This checklist does not decide whether something is legally harassment. It helps you organize what you experienced so a consultation or report is easier to prepare. Nothing you tick is stored or sent anywhere — this page runs only in your browser.
How to use it
Tick what you have experienced, then write a dated note for each ticked item: when, where, what was said or done, and who was present. Bring that record to the Labor Office or an appropriately licensed professional.

The anti-harassment provisions of the Labor Standards Act generally apply to workplaces with 5 or more employees. Smaller workplaces may need different routes — confirm with the Labor Office or an appropriately licensed professional.
